General provisions
Dental care to the population is provided on a territorial basis in the healthcare institution "5th city Dental clinic" in accordance with the legislation of the Republic of Belarus.
Citizens of the Republic of Belarus residing in the service area of the institution have the right to receive dental care within the framework of state guarantees of medical care.
Dental care is provided by dental internists, dental surgeons and other specialists of the institution in accordance with the patient's treatment profile.
Securing citizens registered in the dental clinic's service area
In order to assign citizens to the healthcare institution "5th City Dental Clinic", registered on the territory of the institution's service, it is necessary to personally contact the registry office of the polyclinic: at ul.Voronyansky 50/2, weekdays: from 07:30 to 21:00, Saturdays: from 08:30 to 15:00. When applying, you must have an identity document with you. These documents include:
- passport of a citizen of the Republic of Belarus;
- residence permit in the Republic of Belarus;
- refugee ID card;
- identification card of a citizen of the Republic of Belarus;
- biometric residence permit for a foreign citizen in the Republic of Belarus;
- a biometric residence permit for a stateless person in the Republic of Belarus.
The attachment is carried out in the personal presence of the citizen and the provision of the necessary documents.
Temporary assignment of citizens to dental care
The procedure for providing medical care to citizens of the Republic of Belarus outside their place of residence (place of stay) is determined by Resolution No. 229 of the Ministry of Health of the Republic of Belarus dated 12/22/2008 "On approval of instructions on the procedure for providing medical care to citizens of the Republic of Belarus outside their place of residence (place of stay) and Invalidation of Resolution No. 29 of the Ministry of Health of the Republic of Belarus dated 06/10/2002".
In order to be temporarily assigned to the 5th City Dental Polyclinic healthcare institution, a citizen must personally contact the registry office of the institution, having an identity document with him. These documents include:
- passport of a citizen of the Republic of Belarus;
- residence permit in the Republic of Belarus;
- refugee identity card;
- identification card of a citizen of the Republic of Belarus;
- biometric residence permit in the Republic of Belarus of a foreign citizen;
- biometric residence permit in the Republic of Belarus of a stateless person.
When applying, the patient fills out an application for temporary assignment for medical care (the application form is issued by the registrar upon application). The decision on temporary assignment of a citizen of the Republic of Belarus outside of his place of residence (place of stay) for receiving dental care is made by the head of the institution or a person authorized by him, taking into account the medical and diagnostic capabilities of the institution, the availability of specialists, etc.
Procedure for providing dental care
Dental care in the institution is provided in accordance with clinical protocols approved by the Ministry of Health of the Republic of Belarus.
The guaranteed volume and types of free medical care are provided to the population in accordance with the legislation of the Republic of Belarus and territorial programs of state guarantees for providing medical care to the population.
Additional types of dental services may be provided on a paid basis in accordance with the current legislation of the Republic of Belarus.
